Opt-In Overview
Care recipients consent to receive SMS messages from Checkmate AI by signing and dating a physical SMS Text Messaging Opt-In Agreement. This form is provided by Checkmate AI and distributed to care recipients through their care organization at the time of enrollment.
Caregivers enroll care recipients through the Add New Care Recipient page in the Checkmate AI platform. Before enrollment can be completed, the caregiver must attest that a signed opt-in agreement has been received from the care recipient. This attestation enables a Verify Text button, which sends a confirmation message to the care recipient’s phone number.
The care recipient must reply YES to activate SMS messaging. No messages are sent until this confirmation is received.
Message frequency varies based on care needs. Message and data rates may apply. Recipients can reply STOP to opt out or HELP for assistance at any time.

Care recipients sign and date a physical copy of this form to consent to receive recurring automated SMS wellness check-in messages from Checkmate AI. The signed form is retained by the care organization for compliance purposes in accordance with our Terms Of Service. Consent is optional and is not a condition of receiving care services.

When enrolling a new care recipient on the app, caregivers must attest that a signed SMS Text Opt-In Agreement has been received before a verification message can be sent. The care recipient must reply YES to the verification message to activate SMS messaging. The name in the confirmation copy dynamically changes to match what was entered in the enrollment page. Here, Jane Doe was used as an example.
